Award Process
Intent to Apply

Completing the Intent to Apply form establishes an organization's intent to pursue the award process and confirms the organization's eligibility to receive an award.

• Click here to download the Intent to Apply form. This is a Microsoft Word document which must be saved to your computer, and completed "off-line."

• Complete form and submit by April 1, 2008. If mailed, the form must be postmarked by April 1, 2008.

• Mail the completed form to:

• All applicants are strongly encouraged to send at least one member of their team to DQA Examiner Training. Scholarships are available for all those willing to serve on the 2008 Board of Examiners. Click here for more information on Examiner Training.
