Site Participation Agreement Form

Thank you for agreeing to proctor an exam for a UD Online student. If you have not already done so, please complete the Site Participation Agreement Form and submit to the UD Online office (instructions are included with the form).

Proctor guidelines

Prior to administering the exam

  • Receipt of the examExaminations for UD Online students will be mailed directly to you from the UD Online Office. You will receive the examinations prior to a course instructor's recommended testing date. Upon receipt of the exam, please open it and review the examination cover sheet for time limit, specific exam instructions, materials allowed, etc.
  • Arranging the examIt is the responsibility of the student to contact you to make the necessary arrangements for taking the examination.
  • Student identificationAll students must bring some form of identification with them to the exam. A driver's license or some form of photo I.D. is mandatory. Students who do not have photo ID should contact the UD Online office immediately. The proctor must verify the student's identification before allowing him or her to take the exam.
  • Exam location -- The examination should be given to the student in a quiet, well-lit room. If the proctor cannot be in the exam room at all times, he or she should check with the student frequently for observation.
  • Exam timeline -- Please adhere to the specific exam timelines for UD Online students. An exam should not be administered to a student outside of the permitted exam timeline without specific instruction from UD Online staff.

The examination should be administered as follows:

  • It is required that the examinations be administered at the work site, college testing center, or institution of the proctor.
  • Remove the examination from the envelope in the presence of the student. Instruct the student to read exam instructions carefully.
  • Unless additional materials, notes, etc. are permitted as indicated on the exam cover sheet, the student should only have pens/pencils and blank paper in his/her possession.
  • Note the time the student begins taking the exam with the student so that time limits are adhered to.
  • At the end of the exam, the student must return the exam questions, answer or scantron sheet (if applicable) and any scrap paper used.
  • Return the exam questions, exam answer sheet and scrap paper to the UD Online office in the self-addressed envelope provided. The proctor, not the student, must mail the exam. It is not necessary to photocopy the exam or the student's answers. However, please keep a record of the date the exam was mailed. If the student has any questions, please have him/her note them and enclose them with the exam. (Homework/assignments may be included with the exam.)
  • If a student is observed cheating, all examination materials should be collected and returned immediately with a letter detailing the incident.
  • Exams must be taken during the pre-arranged appointment time. If a student does not "show up" or does not call to reschedule an exam by the end of the exam week, please return the (blank) exam in the postage paid envelope provided with the exam. The exam will be sent back to the professor as a "No Show."
